Head to head is the tiebreaker and has been for quite a while. However, it's not really factored in until teams have played twice.
We are 4th, man. Even the official Serie A website has us as 4th.

I personally don't know if there is an "official rule" about it, but most media, at least in Italy, seem to agree in doing like this:

I see nothing that they say head to head result only count after home and away game played. Nothing about it.

If you look at the standings (right side of the pages in first and second example), they place Inter 4th and atalanta 5th. I read an article on gazzetta about it some months ago and they explained they take into account goal difference before the return match is played. But again, maybe there is no official rule about it.
